By far, the largest and most experience of dental benefits
carrier in the Country. Delta Dental covers nearly one-third
of Americans. Delta Dentals’ customer service far exceeds
industry standards by resolving most inquiries under 15
seconds and on the first call. Delta Dental guarantees
service excellence! PANTHEOS has two plans available for
employees to choose from:
High Plan PPO
This PPO program allows participants to go to any in or
out-of-network general or specialty dentist at the time of
treatment. The PANTHEOS Group dental enrollees have access
to two networks, Delta Dental PPO and Delta Dental Premier.
- Annual maximum of $1,500 per calendar year
- Routine exams and cleanings twice per benefit year
- Bitewing twice a year, Full Mouth every 3 years
- Basic $ Major services at 80% to 50% of reduced fees after
deductible of $50/person or $150/family
- Orthodontia at 50% reduced fee to lifetime maximum of
$1,500
PPO Rates:
- Single: $26.35 per month
- Single+Spouse: $52.69 per month
- Single+Children: $62.09 per month
- Family: $112.68 per month
DHMO Plan
Selecting a primary dental provider for all your dental care
and receiving referrals for specialty care allows enrollees
to minimize their monthly premium expense while experiencing
minimal or no co-payments, no deductibles and no annual
maximums!
- Routine exams and cleanings twice per benefit year
- Bitewing twice a year, Full Mouth every 3 years
- Other services outlined in Plan Detail
DHMO Rates:
- Single: $16.89 per month
- Single+Spouse: $31.23 per month
- Single+Children: $35.28 per month
- Family: $49.65 per month

Voluntary Term Life
- Coverage in $10,000 increments up to 5x annual salary
- Minimum coverage of $10,000 up to a maximum of $300,000
- Guaranteed acceptance to $300,000 for all new employees under the
age of 70.
- Spousal coverage in $5,000 increments up to 50% of the
employee elected amount with maximum coverage to $150,000
up to age 65
- Dependent child coverage of $10,000 for ages 6 months to 19 years
Voluntary Short Term Disability
- Elimination period: 1st day for injury, 8th day for
Illness (including maternity)
- Maximum benefit periods: 13 weeks for employees under the
age of 70
- Maximum Weekly Benefit up to 60% salary, not to exceed
$1000 per week
Voluntary Long Term Disability
- Elimination period: 90 days
- Maximum Benefit Period: To age 65
- Maximum Monthly benefit up to 60% of salary not to exceed
$5,000 per month
Since pricing is dependent upon age and salary, premiums
will be calculated based on your interest in the plan. |

pre-tax savings to reduce you and your families cost to

Healthcare: $5000 plan year max (Jan to
Dec) for healthcare expenses including co-pays, deductibles,
prescriptions and over-the-counter medications.
Dependent Care: $5000 plan year max (Jan to
Dec) for dependent expenses for babysitting, nanny services,
before & after school programs or daycare. Can also be used
for care expenses related to disabled children or parents.

(allows employees to elect to have their claims paid via
direct deposit)
All Flexible spending accounts
require re-enrollment EVERY plan year. Benefits and debit
cards will automatically terminate on 12/31 of each year
unless a new enrollment form is received for the next plan
year. The plan year runs from 1/1 to 12/31. If you decide
not to re-enroll, you have 90 days to file your final
claims. All claims must be received by 3/31 or your account
balances will be lost.
